Job Description:
HCA Florida North Florida Hospital, a cornerstone of healthcare

dedicated and skilled RN Case Manager to join our team. This
critical position supports our mission to offer superior healthcare
by coordinating patient care processes, ensuring the delivery of
cost-effective treatment while maintaining quality care standards.
The RN Case Manager will be instrumental in managing patient cases
throughout the hospital, working collaboratively with physicians,
nurses, and other healthcare professionals to develop treatment
plans and coordinate services, ensuring the seamless delivery of
healthcare and optimizing patient outcomes in our state-of-the-art
surroundings. Assess incoming cases and develop detailed nursing
care plans. Facilitate multidisciplinary planning and collaboration
to streamline patient care. Monitor patient status and progress,
adjusting treatment plans as necessary. Ensure compliance with
healthcare regulations and maintaining patients' confidentiality.
Coordinate with healthcare providers to schedule follow-ups and
additional treatments. Educate patients and families on the
healthcare processes, interventions, and future steps. Document
patient care and interventions accurately for medical records and
billing. Strong interpersonal and communication skills. Effective
decision-making and problem-solving abilities. Comprehensive
knowledge of healthcare regulations and policies. Ability to work
effectively in a fast-paced, multidisciplinary team environment.

Certification in Case Management (CCM) preferred.
