is a not-for-profit Florida corporation that provides a wide range
of mental health and behavioral health care services to the
community such as 24-hour emergency services, inpatient psychiatric
services for children, adolescents, and adults as well as
outpatient services such as medication management, case management,
and counseling. Job Summary The System of Care (SOC) Specialist
serves as the central coordinator for MHRC programs and community
resources, streamlining internal and external referrals to ensure
efficient access to integrated, person-centered care. This role
strengthens care transitions by linking individuals to appropriate
services, monitoring referral outcomes, and addressing service gaps
to support timely follow-up and engagement. By collaborating with
both MHRC staff and external partners, the SOC Specialist helps
prevent unnecessary Baker Act admissions and hospitalizations
through proactive intervention and timely connections to essential
services. As the central point of accountability for agency
referrals, the SOC Specialist ensures prompt service linkage,
stabilizing individuals in community settings and preventing
crises. The specialist also educates stakeholders on available
resources, streamlines care coordination, and enhances continuity
of care, leading to reduced re-hospitalization rates and better
community-based outcomes. Responsibilities Care Coordination
Services: Acts as the centralized coordinator for all referrals,
ensuring individuals are connected to appropriate MHRC services or
external supports in a timely and efficient manner. Tracks and
follows up on referral outcomes to ensure no gaps in service
delivery. Facilitates collaboration across behavioral health,
medical, and social services to holistically address the
individuals needs. Builds trust and rapport with individuals and
their natural supports through proactive communication and
follow-up. Encourages the involvement of chosen supports to foster
recovery and community stabilization. Utilizes structured tools and
criteria to ensure referrals align with evidence-based care
pathways. Ensures seamless transitions between service providers by
facilitating warm handoffs and maintaining continuity of care.
Advocacy and Problem Solving Identifies and addresses Social
Determinants of Health (SDOH) barriers such as transportation,
access to identification, and housing. Serves as an advocate for
individuals by ensuring access to appropriate services and
resources, addressing barriers to care, and supporting timely
resolution of coordination challenges. Monitors referral outcomes,
analyzes data trends, and identifies areas for improvement to meet
quality and performance standards. Hospitalization Prevention and
Early Intervention Identifies high-risk individuals and connects
them to preventive services and supports to reduce unnecessary
hospitalizations. Works with multidisciplinary teams to standardize
care coordination practices, address service gaps, and support
MHRCs mission of community stabilization. Community Engagement and
Outreach Serves as a liaison to community partners, providing
education on MHRC services, referral pathways, and available
resources. Engages in outreach activities to strengthen
partnerships and build awareness of MHRCs comprehensive services.
Continuous Improvement and Compliance Participates in quality
improvement initiatives to enhance referral processes, reduce
delays, and align with best practices. Ensures all documentation is
completed accurately and timely, meeting organizational and
regulatory compliance standards. Qualifications In order to be
considered, a candidate must have a Bachelor's Degree in Social
Work or a related Human Services field from an accredited
university or college (a related Human Services field is defined as
one in which 30 hours of course work includes the study of human
behavior and development) required. One year of experience working
in human services or a mental health related field required.
Experience working with adult individuals with mental illness
preferred. Proficiency in the RBHS/MHRC Electronic Health Records
(EHR) and Patient Information System demonstrated within three
months of employment. Must complete competencies assigned to
position as outlined in the Orientation Competency Form.
Proficiency in Microsoft Office, Outlook and use of the Internet
required. Strong communication skills are essential and this
individual must be able to interact appropriately with internal and
external customers, including patients, families, caregivers,
community service providers, supervisory staff and other department
professionals. Position Details This is a Full Time Days position:
Monday through Friday, 8:00am to 4:30pm. Renaissance Behavioral
