Job Description Join Our Team and Make a Difference in Post-Acute Care ?We are seeking a compassionate and experienced Family Medicine Physician to join our dynamic Post-Acute Division. This full-time position offers a rewarding opportunity to provide high-quality care to our skilled nursing facility patients in the St. Augustine and Jacksonville areas. Family Medicine Position Responsibilities: Rounding: - Monday-Friday rounding on 1-2 buildings per day, depending on census. - NP support to assist with patient care. Patient Care: - Manage a patient load of 20-22 patients per day. - Utilize Nursing Home EMR for efficient charting. - Provide comprehensive care to skilled, short-term patients with 3 visits per week. - Attend to 3-5 buildings as needed. On-Call: - Day call coverage from 8 AM to 6 PM for facilities rounded on that day. Benefits and Compensation: - Competitive Compensation: $180K-$220K annual salary - Productivity Bonus: Additional earning potential - Generous PTO: 15 days of paid time off - Comprehensive Benefits: Medical, dental, vision, and 401K through UnitedHealthcare - CME Stipend: $2500 annually to support professional development
