This crucial role involves overseeing patient care, ensuring a smooth and effective discharge process, and maintaining our commitment to improving human life. The ideal candidate will have expertise in patient handling, discharge planning, and a strong ability to communicate with both patients and healthcare teams. Manage the discharge process of patients to ensure a smooth transition from hospital to home or other care facilities. Collaborate with healthcare team members to develop discharge plans that meet the specific needs of each patient. Educate patients and their families about post-discharge care procedures and any necessary follow-up. Perform clinical tasks such as monitoring vital signs, administering medication, and providing necessary health interventions. Document all patient interactions and interventions in the patient healthcare record accurately. Address any concerns or questions from patients and families regarding the discharge process. Strong nursing skills and knowledge Excellent communication and interpersonal skills Ability to work collaboratively in a team environment Efficient in time management and multitasking Proficient in electronic health record (EHR) systems Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid and active Registered Nurse license BLS Certification from the American Heart Association or a similarly recognized provider
